Toothville Family Dentistry is committed to saving you time, which is why we have online forms you can download right from our website. Using the links below, the necessary paperwork for adults and children can be downloaded as PDFs and filled out at your convenience, saving you time at your first appointment.

Our new patient forms are fillable and signable with Adobe PDF Reader. You can download your chosen form, type directly into the fields, and then print out the completed form to bring with you to your appointment. Or if you prefer, you can also save the completed form on your computer as a PDF and email it to us at info@toothville.ca. When you arrive at your first appointment, you will be warmly greeted by one of our Toothville Family Dentistry Team, and offered coffee or water. If you haven’t filled out your new patient registration forms before you arrive, we will ask you to do so and get a copy of your insurance card.

We always try to keep your appointments as efficient as possible, ensuring that it starts on time and ends on time. Waiting times are kept to a minimum. We’ll promptly bring you back to the dental chair, where you’ll meet with Dr. El Gamal or Dr. Al-Gertani for your examination. You will have an opportunity to discuss your oral health goals, and digital X-rays and intraoral and extraoral pictures will be taken.

Together, we’ll review these images and create a treatment plan to help you achieve your dental goals. If applicable, you can have your cleaning completed as well, with our dental hygienist. You can then check out at the front desk, where we’ll discuss insurance coverage and costs with you. If you require more complex treatment in the future, our manager and treatment coordinator will discuss your payment options.

We always like to keep in touch with our patients, and we are happy to do so by whatever technology you feel most comfortable with. Whether by email, text message, or phone call, we will make sure it fits your schedule to help you stay informed about your dental health goals.
